You have the right to dispute the accuracy of the information PERS used to calculate your retirement benefit. If you feel the information used to calculate your benefit is inaccurate, complete the Dispute of Notice of Entitlement form (available by clicking the Form button). Important: PERS must receive the signed form either within 240 days of the date PERS mailed your Notice of Entitlement or by the due date of your first benefit payment, whichever is later.
