Upon taking a distribution from OSGP, the third-party administrator (TPA) will issue you a Form 1099R
. Use this form when you file your taxes in the year following the year of the distribution.
To get a duplicate 1099R form, follow these steps:
| 1 | Go to http://cms.oregon.gov/pers/OSGP/Pages/index.aspx, and then log onto your account. |
| 2 | Go to My Correspondence & Records in the upper right corner of the screen, and then click on the Tax Forms link. From here you can print the form. |
