Apply within six calendar months from the date you last worked for a PERS-participating employer. Continuous disability from last day worked is not required. If you don’t apply within the six months, you may apply within five calendar years from the date you last worked for a PERS-participating employer; however, you must show continuous disability from your last day physically on the job with your PERS-participating employer, and you must not have worked in any other capacity since your PERS-participating employment.
