If you are an:
• | Active member working for the state of Oregon and currently contributing to OSGP, change your address through your Human Resource Department. |
• | Active member who works for the state of Oregon but who is not currently contributing to OSGP, complete the Address/Name Change Form and return it to OSGP. |
• | Active member working for a local government or school district, higher education, or a miscellaneous state agency (either contributing or not), complete the Address/Name Change Form and return it to OSGP. |
• | Alternate payee, retiree, or beneficiary, complete the Address/Name Change Form and return it to OSGP (accessible by clicking on the Form button). |
