If you are married on your effective retirement date or there is anyone who is constitutionally required to be treated in the same manner as your spouse for purposes of retirement benefits, the pension payable to you will be as provided under the Half Survivorship Benefit Option with your spouse or the other person as the beneficiary. This rule applies unless (1) you have chosen the Full Survivorship Benefit Option with the spouse or other person as the beneficiary or (2) you submit a notarized consent form signed by your spouse or the other person consenting to a different option or beneficiary.
This requirement does not apply to a pension benefit that is paid in a lump sum because the monthly benefit is less than $200.
